A handful of insurance plans require a physician referral for the audiologist visit to be a covered benefit, so it's worth a quick call to your carrier before your appointment if you want to be certain.
If your primary care physician or an ENT has already referred you, bring the referral paperwork and any recent medical records — your audiologist will have the full clinical picture from minute one.
Bring a photo ID, your insurance card, a current list of medications, and any previous hearing test results or audiograms you can find. We'll have your intake paperwork ready at check-in, so arriving 15 minutes early gives you plenty of time to finish before your evaluation begins.
